After extensive negotiations, the Trump administration’s sweeping OBBBA was passed, introducing major changes to healthcare programs that will reshape Medicaid, Medicare, and the ACA.

Medicaid reforms aim to reduce fraud, improve enrollment processes, curb wasteful spending, prevent abusive financing, promote personal accountability, and expand access to care. Key changes include repealing Biden-era eligibility and nursing home staffing regulations, implementing work requirements, sunsetting the American Rescue Plan’s temporary FMAP increase, and limiting state taxes on providers.

For Medicare, the bill reduces immigrant eligibility, updates the Physician Fee Schedule conversion factor, and expands the list of drugs exempt from the Medicare Drug Negotiation Program.

Regarding the ACA, the OBBBA limits refundable tax credits for undocumented and certain lawfully present immigrants and ends auto reenrollment and provisional enrollment starting in tax year 2028.
